Scalp reduction is an operation in which a section of bald scalp is excised and the hair-bearing skin around it is advanced to close the gap. It treats baldness by making the bald area smaller rather than by transplanting hair into it. It was common decades ago and is uncommon today, because follicular transplantation delivers a more natural result without the scar and the tension that scalp reduction involves. The American Society of Plastic Surgeons publishes national average costs for its surgical procedures but publishes none for hair transplantation of any kind.

Why transplantation displaced it
Advancing scalp under tension leaves a scar within the hair-bearing area, and the tension can cause the scar to stretch and the surrounding hair to thin further. The direction of hair growth on either side of the closure often does not match, which produces a visible seam. Follicular transplantation, described by ASPS as relocating small pieces of hair-bearing scalp graft by graft, avoids both problems and is what most surgeons now offer instead.
Where it still has a role
The operation retains a narrow place for closing an area of scarred or permanently hairless scalp, for instance after burns or after excision of a lesion, where the aim is reconstructive rather than cosmetic. In those cases it is usually assessed as reconstructive surgery, which is a different conversation with an insurer than a cosmetic one. Whether your case falls on that side is a clinical judgement that has to be documented, not asserted.
What to do with a quote for it
With no published average and few surgeons performing it, a quote for scalp reduction cannot be benchmarked. The useful step is to ask the surgeon why they are proposing this rather than transplantation, what the scar will look like and where it will sit, and what happens if the hair around the closure thins further with age. If those answers are not specific, seek a second opinion from a surgeon who does transplantation.
Questions people ask about scalp reduction surgery
What is scalp reduction surgery?
An operation that cuts out an area of bald scalp and advances hair-bearing scalp to close the gap, making the bald area smaller.
Is it still performed?
Rarely for cosmetic baldness. Follicular transplantation largely replaced it. It retains a role in reconstructive cases such as scarred scalp.
What does it cost?
No national average is published. ASPS publishes no cost figure for hair restoration procedures at all.
What are the drawbacks?
A scar inside the hair-bearing area, tension that can stretch it and thin nearby hair, and a mismatch in hair direction across the closure.
